APLLICATION TO SUPPORT THE RECLASSIFICATION FROM LEVEL RN 1 TO LEVEL RN 2 (4 CRITERIA/STANDARD NEEDS TO BE ADDRESS): I WORK IN A PUBLIC HOSPITAL AS A CARDIAC CATH-LAB/recovery SENIOR NURSE

             For each criterion/standard, the applicant should respond by considering these two simple question:

             What have I done in my nursing practice to show that I meet this criterion?

             What evidence can I provide, if requested, to show that I meet this criterion?

1. CO-ORDINATION OF SERVICE (150 WORDS)

• Evidence by way of example from client/patient and peer related to the quality of your care provision-peer comments
• Example of level of involvement in patient education, counselling and programs
• Evidence by way of example of role in evaluating patient care outcome
• Overseeing the provision of nursing care within a team or theatre

(Points to be involved: effective communication with all members, patient’s hand over to relevant staff, liaise with patient and family member, safe discharge process and arrange treatment follow up, checking Cath labs and patient lists in beginning of the shift)

2. QUALITY AND SAFETY (100 WORDS)

• Evidence by way of involvement in any clinical auditing (mention about discharge planning and radiation safety audit for the unit)
• Embrace and support new safety initiatives (manual handling facilitator for the unit)
• Evidence by way of example of involvement in policy review and quality improvement actives (Medication safety nurse for the unit and look after ALS TROLLEYS in tech suits or theatres).

3. Team Leadership (150 words)

• Shift team leader
• Supporting less experience staff
• Involvement in meetings and procedures and policy development
• Evidence by way of example of involvement in incident reporting, evaluation and management.
• Management and resolution of a conflict issue/situation
• Evidence by way of example of involvement on unit/team service planning and evaluation
• Contribute to communication processes that effectively deal with challenging behaviours and the resolution of conflicts

4. Clinical Teaching (150 words)

• Participate in clinical teaching, overseeing learning experiences, and goal setting for students, new staff with less experience ( Preceptor for new nursing staff and student nurse for the unit)
• Professional portfolio responsibility/participation ( medication safety nurse, stroke management nurse for the unit, manual handling instructor/facilitator for staff and unit)
• Participation in workshops and education sessions for staff
• Evidence by way of example of how you maintain and develop your knowledge base and how that impacts on patient outcomes
• List resource used regularly to update knowledge evidence by way of example of how you achieve client/carer involvement in determining care management and outcomes.
