Your patient
Select a patient/client you have recently cared for who required a pharmaceutical intervention. They may be complex or simple- it is more important that your answers are very detailed about one medication rather than superficial about several.
How many medications?
I recommend a minimum of two medications so you can demostrate any issue around their use together. Two to three medications is preferable.
More than three may be too complex, but will depend on the case. You will notbe penalised for too many medications but please ensure you can demonstrate the relevance to your case and that you use the marking criteria as this will help you keep within the boundaries marking.
It may be more prudent to choose one or two medications and list the remaining medications but this is up to you.
Patient centred care
Demonstrate within the case study at least one strategy that involves the patient/client as a partner in management decisions. This may be simply noting that you have asked the patient if they feel the effect of the drug.
Don’t forget rationales relevant to your case
Please describe the rationale for the use of the medication by using detailed knowledge of pharmacology, and link the patient condition with pharmacokinetics and pharmacodynamic principles as it applies to your case. For example, the dose may be higher for an obese patient or lower for a patient with hepatic disease if the drug is metabolised in a pathway that may be compromised.
Remember patient safety aspects and make at least one reference to this in your case study response to Item 5
Describe the comprehensive health assessment that was attended- for example NSW Adult Observations, or Glasgow coma scale, pain scales etc, .imaging, blood workups etc, outlining the diagnoses and health problems.
Document the interventions including medications that the patient is currently receiving. Don’t forget IV fluids.
Provide a rationale for the need for medication therapy, including consideration of non-medication treatment. Suggest non-medication alternatives that are evidence based, or patient preference where relevant.
Describe the pharmacokinetics, pharmacodynamics and formulation of the medication(s). Explain the effects on the individual patient and potential unwanted effects, the monitoring required and therapeutic endpoints for therapy. Suggest how the medication regimen might be optimised based on pharmacological principles.
Use current evidence- must be within last 5 years
Use unbiased sources of drug and therapeutic information and the evidence base which supports the decision to prescribe a particular drug for a particular patient. At each step along the prescribing pathway you will need to support each decision with an evidence based source.
