Tenerife, Canary Island – In this 1977 disaster, on take off a KLM Boeing 747 collided with a Pan Am 747 that was on the active runway; 583 lives were lost, 61 survived.
Air France Flight 447 – In this 2009 disaster, an Airbus A330 plummeted into the Atlantic, 228 lives were lost.
In addition to these two disasters, you will select a disaster or near disaster of your own choosing where culture in the cockpit or between the tower and cockpit was a factor.

Your assignment is to write a comprehensive analysis in which you will:
Summarize the factors and situations that led to the crashes.
What mistakes were made?
Who made them?
Were the problems the result of crewmembers doing the wrong thing or nothing?
Were there conflicts among the crew or between the crew and the tower?
Were there any international differences in training, standard procedures, or regulations involved?
Describe what, if anything, crew or controllers might have done differently to avert the disaster.
Determine how culture in the cockpit and/or in the tower may have been responsible for all or part of what happened.
Report on any changes that were made at the airport or by the airlines to avert future similar accidents.
Report what precautions you will take when faced with potentially dangerous situations.
Include answers to the following questions in your report along with any other information you feel is essential.
