NSB231 Integrated Nursing Practice 2 (On Campus) Case Study
Task description: For this essay, select either Essay Case Option 1 (the paediatric case) or
Essay Case Option 2 (the adult case). Review your chosen case and the
clinical documents. Using the clinical reasoning cycle you are required to
assess, plan, implement and evaluate your care for the patient. After
identifying the relevant assessment data you must identify two (2) priority
problems for your patient. For each priority problem identified, list two (2)
nursing interventions (collaborative/independent) linking to your research
of the literature. Include a one to two line rationale for each intervention.
Evaluation of your care is essential.

• Conclusion (150 words approx): Briefly restate the patient’s
condition and problems and how the nursing interventions can
benefit the patient’s health.
• You need to discuss at least one independent nursing intervention
in your essay.
 For any collaborative nursing interventions you must discuss the
nursing responsibilities and actions in relation to this collaborative
intervention.
 Medications cannot be both of your chosen collaborative interventions
for a problem. If a medication is one of your collaborative
interventions you must identify an appropriate drug that is likely to
be/or has been prescribed by the doctor. You must consider and
discuss the nursing responsibilities associated with the medication
(action, dose, side effects, prevention and management of same,
administration requirements, evaluation).

CASE 1

Matthew is a 14 year old adolescent with Cystic Fibrosis.
He was visited at home by the Hospital in the Home team
for IV antibiotic therapy received via his portacath. Matthew
has a frequent productive cough. He tells you that his
sputum has changed from yellow to greenish in colour
recently. His respiratory rate is 22 breaths/min and he has
crackles upon auscultation. His temperature is 37.6 C.
Matthew’s mother confides that she is worried about her
son as he hasn’t been engaging in as much aerobic sport
and physio lately and she has seen Matthew discarding
tablets (pancrease capsules) in the rubbish bin. When you
weigh Matthew you note that he has lost 2 kgs of weight
since your last visit 2 months ago. His weight is 47kgs and
his height is 168cms. You speak with Mathew’s doctor who
organises his admission to hospital.

CASE 2

Beryl is a 50 year old woman with Hypertension who is
attending a Healthy Heart Workshop at her local
Community Health Centre. Beryl’s blood pressure is
160/95, she is 150cms in height and her weight is 105kg.
Beryl reports that she drinks two to three glasses of wine
per night, eats a lot of take away foods as she’s too busy
with her executive management job to think about cooking.
She is a single mother caring for three school aged children
to always cook. She states that she rarely exercises due to
lack of time. Beryl visited her GP last week after feeling
unwell. Pathology was taken and Beryl’s GP report shows
that her total cholesterol is 6 mmol/L, Low Density
Lipoprotein 3.1 mmol/L and High Density Lipoprotein
0.8mmol/L. The GP commenced Beryl on medications but
she states “I keep forgetting to take them, my life is just so
stressful at the moment. I really don’t have time to be here,
I need to be at work”.
